**IMPORTANT NOTE**

This enrollment lottery web form is designed to be used one time only.  If, after completing the web form, you find that you have left a student out or entered incorrect information, you will need to call the school office to get it fixed. Please do not use this web form again if you have already completed it previously.

Also, please note that using the back button on your web browser is not recommended.  Doing so can cause duplicate records and/or the inability for you to complete the web form process.
